This discharge plan is of a 58-year old male patient with a family history of high blood pressure and high cholesterol. He also presents with complaints of chest pain. The patient likely has a cardiac involvement, and the discharge plan will aim at preventing cardiovascular diseases such as heart failure, myocardial infarction, and hypertension. In the discharge plan, I will first identify the patient’s primary caregiver, who will help the patient in performing activities of daily living (Brown, 2018). I will provide health education to both the patient and his caregiver on the prevention of future hospital admissions by modification of lifestyle and adherence to the medication regimen (Brown, 2018). Lifestyle modification will entail interventions to maintain a healthy weight, such as engaging in daily physical exercise and consumption of a healthy and balanced diet. I will inform them that lifestyle modification is essential since it helps in lowering blood pressure and preventing chronic diseases such as heart failure, atherosclerosis, and diabetes.
