Quite often, nurse leaders are faced with ethical dilemmas, such as those associated with choices between competing needs and limited resources. Resources are finite, and competition for those resources occurs daily in all organizations.

For example, the use of 12-hour shifts has been a strategy to retain nurses. However, evidence suggests that as nurses work more hours in a shift, they commit more errors. How do effective leaders find a balance between the needs of the organization and the needs of ensuring quality, effective, and safe patient care?

In this Discussion, you will reflect on a national healthcare issue and examine how competing needs may impact the development of polices to address that issue.

To Prepare:

1. Review the Resources and think about the national healthcare issue/stressor you previously selected for study in Module 1.

2. Reflect on the competing needs in healthcare delivery as they pertain to the national healthcare issue/stressor you previously examined.

For the Discussion : Post an explanation of how competing needs, such as the needs of the workforce, resources, and patients, may impact the development of policy. Policy development plays an influential impact in the organization’s work environment and the community it serves. While competing needs are essential to consider when developing policies, they can potentially impact the policy’s maturity due to the position of the policy and what it is supporting. For example, when it comes to appropriate staffing ratios, the organization must consider the patient and staff needs while also reflecting on the financial aspect of the cost. In this competitive world of healthcare, most organizations strive for positive patient experiences because it is a direct driver of performance for hospitals (Ozcelik et al., 2021). Therefore, the five P framework is a fantastic example of how hospitals consider the competing needs and their impact on the development of policies. The five P’s look into multiple aspects of the organization to make assessments for appropriate policy development and focus on the purpose, patients, professionals, processes, and patterns that help guide analysis and evaluation for organizational growth based on competing needs.

A hospital’s primary goal is to support those who deliver direct care to the people or communities served by the organization; therefore, policies addressing nurses must be considered of utmost importance (Marshall & Broome, 2021). Several competing needs such as aging workers, specific region growth, burnout, workplace violence, and technology play a significant role in the nursing shortage. As the Northeast and Midwest region’s population growth declines, the West and Mountain regions are projected to rise, indicating a need for more nurses to combat the higher retirement population (Haddad et al., 2020). The overwhelming frequency of increased workplace violence and the organization’s lack of response to adequately protect nurses and their physical and emotional well-being contributes to staff retention.

Focusing on a competing need that is continually going to affect nurses in the profession is technology. The impact technology places on the nursing shortage is essential to note. Studies have shown that nurses spend 32.4% of their time on electronic devices, suggesting that time with patients declines as the need for technology increases (Sun & Cato, 2020). Although technology is a competing need that will not end, the policy can help address this issue by implementing interoperability. Interoperability in healthcare is the ability to exchange data within an organization to optimize time management. For example, if the organization would employ the interoperability of smart pumps, that would decrease the time needed for nurses to input intravenous medication rates in the chart. Therefore, interoperability would increase the time for excellent bedside care and reduce nurses’ stress to ensure proper patient safety, encouraging nurses to stay. A policy can also address the competing need of the aging population by utilizing evidence-based practice (EBP). As the population is living longer, an increase in health services is becoming more prevalent. Treating these prolonged illnesses can add unneeded stress to the workplace and the already short nursing population. Therefore, the practice of EBP can explore the potential modification to workflows, consequently improving patient outcomes and promoting nurses’ autonomy, which research has shown ultimately increases the likelihood of nurse retention.
