The most surprising statement from the NAMS videos is that “women’s sexual dysfunctions are real.” It was surprising since I only knew of sexual dysfunction in males and not in females. I was further surprised to hear that up to 80% of women will have symptoms of Genitourinary Syndrome of Menopause (GSM), with 50% of them having sexual dysfunction. GSM is a collection of physical changes and symptoms such as vulvovaginal dryness, irritation or burning, dyspareunia, and urinary symptoms including dysuria, and recurrent urinary tract infections. The physical changes and symptoms are associated with estrogen deficiency.

The genitourinary system is involved with profound symptoms in the lower urinary tract and vaginal and perineal tissues. On the other hand, I feel uncomfortable asking male patients if they have concerns about premature ejaculation. This week’s information has equipped with knowledge on screening of GSM in postmenopausal women, breast cancer-therapy patients, and breast cancer survivors. In my future practice, it will be a norm to question postmenopausal women on the presence of sexual and urinary symptoms to rule out the presence of GSM.
